CONFIRMATION/INVOICE: The CONFIRMATION/INVOICE will be sent via fax or email.

SPONSOR: Individual(s) or Entity conducting the hole-in-one contest. SPONSOR agrees to comply fully with all terms and conditions as detailed in the
UNITED HOLE-IN-ONE AGREEMENT.
